This requires experience in senior needs, social work, nursing, or counseling.
Case Management Services:

- Help a client designate a health care proxy and compose a living will
- Coordinate and monitor in-home health care and services to include: physician visits, physical therapy, pharmacy ordering and delivery, and hospice services
- Identify problems and services necessary to rectify them
- Offer a professional, objective point-of-view regarding senior care options
- Save families time and money by eliminating duplicative and unnecessary services
- Utilize trusted resources to make physician, mental health, attorney, trust officers, and housing referrals
